Initial Visit

Please arrive 10-15 minutes early to fill out the new patient forms and health questionnaire.  You can also download and fill out the forms prior to your visit to save you time. Please remember to bring the following:

  • Driver’s License or State ID
  • Referral if required by your insurer
  • Insurance information
  • Copies of your operation records, medical records, imaging test results (x-rays, MRI, CT scans, etc.), etc. from your previous doctor visits.

Cancellation Policy

In the event that you need to cancel your appointment, kindly provide 24 hours notice. Your consideration in this matter will allow us to accommodate other patients who could benefit from an appointment.

Insurance and Billing

We strive to provide all of our patients with affordable medical care.  In order to do so, we request that payment arrangements be made at the time of services rendered.  We accept cash and all major credit cards (Visa, MasterCard, American Express, Discover) for the convenience of our patients.  We will also bill your insurance company and assist in handling claims in every way we can, but responsibility for payment ultimately falls to the patient.

Our main focus is to provide our patients with the appropriate medical care for their individual needs.  Our billing office will work with you regarding any special needs or concerns you may have.

Office Policies

Prescription Refills

If you require a prescription refill, please contact your pharmacy and ask them to send an electronic request to us.

Please submit your request to the pharmacy at least three days before running out so there is no delay.

Please allow up to 2 business days for the request to be reviewed and submitted back to your pharmacy.

For new prescription request, please call our office at (713) 526-2663.

During evenings, weekends or holidays, routine prescriptions cannot be filled so please plan accordingly and call us during the weekday before your medication has run out.  After 5pm during the week or on the weekend we cannot call in prescriptions for pain medications. Patients will need to make an appointment to see the doctor.  Patients should always visit the emergency room in urgent cases.

Please note that Dr. Brown is not a pain management specialist.  He does not manage chronic pain.  He only prescribes pain medications for recent injuries, post-operatively, etc.

Disability Forms

For disability and other forms to be processed, please drop off the original at our reception desk.  The form will be forwarded to the appropriate staff member for processing and mailed to the address on the form.  A small fee may be assessed for our completion of these forms.  Please allow up to 72 hours for processing.

We will provide copies of health care records upon receipt of proper request form.  You may print and complete the following form:
